Coaching Evaluation Forms
Sault Female Hockey Association coaching evaluations are now live on our website.

Each team manager has been provided with a unique team code. This code will be required at the end of the evaluation form to confirm authenticity. Please connect with your team manager to obtain the code, as evaluations submitted without it will not be considered.
We ask that all feedback be shared respectfully and constructively.
We would also like to take this opportunity to sincerely thank our coaches for the time, effort, and commitment they give to our girls, our association, and our community. Your dedication is what keeps this league running.
These evaluations give you a voice, and help us recognize what is working well and identify areas where we can continue to improve and provide support.
