This form must be completed by all coaches/managers that wish to fundraise for their SFHA team. A new form must be filled out for each fundraising event. Once approved the respective VP (Jr Greyhounds Girls / House League) will notify your team contact and the event may proceed.
I acknowledge that I have read and understood the terms and conditions enclosed in the SFHA Fundraising Procedures. I understand that my failure, or my teams failure to abide by the rules set out by the SFHA may result in disciplinary action and/or suspension from league sponsored activities. I agree that no Fundraising Activities will be commenced without approval of this request. I agree to provide full details of funds raised and expenditures of funds (including receipts) in the Team’s Financial Report.
I acknowledge that the team will not include 'games of chance' during this fundraiser as per the SFHA AGCO agreement.
